Mike Rich
Mike Rich brings a combination of education, experience and vision to the
organization. Mike graduated from Arizona State University in 1990 with a
Bachelor of Arts degree in Education and a minor in Science. It didn’t
take Mike long to establish himself as a defining member of the sports medicine
community. As Head Athletic Trainer at John C. Lincoln Hospital, Mike
proposed and integrated a sports medicine contract for the Paradise Valley
School District. This vision provided athletic trainer coverage for their
sports teams. Later that year he and two partners opened SPORT
Rehabilitation Institute, a physical therapy clinic in Mesa. During that
time Mike became a teacher/athletic trainer at Gilbert High School where he
designed and implemented a sports medicine curriculum in the secondary schools
while overseeing course instruction and the student trainer program. Mike
currently serves as District Athletic Trainer for the Gilbert Unified Schools
and is responsible for overseeing the individual school trainers, for
formulating the district sports medicine budget, and for educating the coaches
in basic sports medicine concepts.
In 1994 Mike and his partners opted to merge their growing number of local
physical therapy clinics with Physiotherapy Associates, a national group based
in Memphis. He served as a Director for the firm until 2007 when his
vision for Restore came to fruition.
Mike has been involved with Mobile Sports Medicine Systems Inc. since
1990. Better known as the Justin Boots Sports Medicine Team, this company
has provided traveling medical coverage for professional rodeo (PRCA), bull
riding (PBR), ski tours (Chrysler pro downhill) and motor sports events (Nascar
and ppg indycar). In addition to the sports already mentioned, Mike has
worked with trainers and physicians in a wide spectrum of professional
sports. He worked with the team trainer and physicians of the Phoenix
Roadrunners of the International Hockey League in the treatment of their
players while being seen at the Lincoln Institute for Athletic Medicine.
In this same capacity he worked with the California Angels and Milwaukee
Brewers organizations with major league baseball. He assisted tour
medical personnel in the recognition, prevention, treatment and rehabilitation
of injuries sustained by professional volleyball players for Phoenix based
tournaments of the AVP Volleyball Tour as well as the Budweiser 4-Man
Volleyball Tour. Mike has also traveled overseas providing medical
coverage with the ATP pro tennis tour.
Mike has served on the AZATA Board of Directors as the Clinical/Industrial
representative for two consecutive terms (91-99), and served two terms as a
representative on the NATA Clinical/Industrial/Corporate Committee.
With seventeen years of experience in medicine from the high school to
professional levels in many sports, and from hospitals to outpatient clinics,
Mike has always enjoyed helping the patients.
It is rare to encounter a person with education, experience, vision, and
compassion. Mike Rich is such a person.

Shannon McLean
Shannon graduated from the University of Arizona in 1996 with a B. S.
in Exercise and Sports Science. She received an M.S. in Physical Therapy
from the University of Miami in 1999. She possesses an intuitive problem
solving ability that is imperative in the practical application of physical
therapy and sports medicine. Open mindedness is essential when confronted
with the myriad of possibilities regarding sports medicine and that has proven
to be Shannon’s strong suit. She has worked in outpatient orthopedics for
the past 7 years; 2 of those years as clinical director. Shannon has been
licensed in Arizona since 2000 and was a Certified Orthopedic Manual Therapist
(COMT) at Maitland Australian Physio Therapy.
While Shannon has experienced several knee injuries and knows about the
importance of effective rehabilitation from a personal and professional level,
she believes in the adage that “an ounce of prevention is worth a pound of
cure”. She plans to utilize Pilates based exercises for flexibility,
strength and body control. She personally participates in local cycling
events such as the “Tour de Tucson” and the MS150 as part of her own physical
conditioning program. Shannon “walks the talk”.
Her experience and diligence have made her one of the foremost authorities on
physical therapy issues in the Southwest. Shannon’s innate problem
solving skill coupled with her energy and dedication to her chosen profession
will certainly bolster her position in the sports medicine community.

Doug Rich
Doug Rich PT, MS, OCS has practiced as a Physical Therapist for over 13 years in the State of Arizona. He is a Nationaly recognized Orthopedic Certified Specialist. Doug recieved his Masters of Science in Physical Therapy from University of Kansas Medical Center in 1994 after receving his undergraduate Bachelors of Science in Microbiology from Kansas State University.
Doug began practice at St. Joseph's Sports Medicine in 1994 which included 2 years at Arizona State University working with students, faculty and athletes. The next 10 years Doug directed and treated patients for a large Outpatient Sports Medicine Clinic located in Scottsdale. Doug's achievements include passing the boards for Orthopedic Certified Specialist, completing over 30 continueing education courses in the field of outpatient orthopedic physical therapy, lecturing at both ASU and for the State Physical Therapy Boards yearly meeting, as well as being elected by his peers in 1994 as representing the top 1% in the field of physical therapy on the bestofus.com website.

Jay Bloomfield
Jay Bloomfield graduated from Northern Arizona University with a Bachelor of
Science in Physical Therapy in 1988. Since that time he has been practicing
orthopedic and sports physical therapy, utilizing a hands-on manual approach
with exercise based enhancement. He has worked with numerous college, Olympic
and professional athletes, including members of the Phoenix Roadrunners,
Arizona Rattlers, Montreal Expos and the San Francisco Giants.
As an ex-college athlete, Jay has experienced many injuries resulting in
multiple surgeries. These experiences have helped to enhance his understanding
of the patient’s rehabilitation to better serve his clients.
Jay has worked directly with the Paradise Valley Unified School District
(PVUSD) since 1990. He managed the PVUSD sports medicine program which trained
and educated the coaches and functionally rehabilitated the athletes.
Jay is married and has twin daughters who attend elementary school in the Cave
Creek Unified School District. He enjoys spending time with his family and
playing golf.

Eric Elmore
Eric Elmore is a native of Scottsdale, AZ. Growing up, Eric participated in
many sports with soccer as his primary interest. Following graduation from
Chaparral High School in 1996, he attended the University of Arizona,
graduating with a Bachelor's Degree in Physiology in 2000. The University of
Wisconsin - LaCrosse followed, from which he earned a Master's Degree in
Physical Therapy in 2002 with an emphasis in sports and orthopedics. Eric
additionally acquired certification as a Certified Strength & Conditioning
Specialist (CSCS) from the National Strength & Conditioning Association
(NSCA) during his time up in Wisconsin.
He moved back to the valley of the Sun and has been practicing sports and
orthopedic physical therapy since, first with PHYSIOTHERAPY ASSOCIATES and now
with JOINT PERFORMANCE. Areas of interst within sports and orthopedic physical
therapy include research (Bailie DS, Ellenbacker TS, Elmore EM. Descriptie
report of shoulder rance of mothion & rotational strength 6 & 12 weeks
